Instructions for Processing Address Change（Include the Domestic Correspondent）and  Specimen Chop Loss Register

1. Address Change（include the Domestic Correspondent）:

On the Application of Address Change, the shareholder shall fill in the name in Chinese and shareholder account number. Additionally, the columns of mailing address and telephone number shall be filled in the local mailing address and telephone number of the domestic correspondent (an adult of R.O.C. citizen) respectively. For example: c/o Chen Ta-ming, No. 166, Sec. 2, Ming-Sheng E. Rd., Taipei City in Chinese. If there is no such domestic correspondent, the shareholder shall fill in his/her own mailing address. The Application shall be affixed with the specimen chop, and the address will be changed after the Stock Affairs Department has verified the specimen chop to be correct. Thus all notices could be received by the shareholder in the future.
2. Specimen Chop Loss Register: The shareholder has lost his/her original specimen chop

(1) Processing in person: The shareholder shall bring his/her progenitor of passport, chop and the stock certificates that the shareholder's name had been registered on the backside to the Stock Affairs Department at 2F, No. 166, Sec. 2, Minsheng E. Rd., Taipei City, Taiwan for processing.
 
(2) In case of processing by mail, the required documents attached are as follows:
  1. Application of Chop Loss Register: Fill in the shareholder account number, name in Chinese and affix new specimen chop in the column of new specimen chop. The column of date is kept in blank.
  2. Specimen Chop Card: Affix new specimen chop, and complete basic information（include address of domestic correspondent or contact Tel. No. of shareholder）
  3. Identification Papers and copies thereof: Passport, Alien Residence Certificate or certificate legalized by an R.O.C. representative body or issued by local court or government agency in the shareholder's home country, or identity document notarized by a statutory notary institution in the shareholder's home country.
  4. Stock Certificate: If the original specimen chop is affixed on the backside of stock certificate, a new specimen chop shall be affixed in the column of transferee.
(3) In case of processing by consigning to a third party: The consignee shall be a citizen of R.O.C.. The ID card of the consignee and a Proxy shall be further attached in addition to the documents mentioned above.
