Relevant Matters about the Dividend Check

Suggestion: For your convenience, you may fill out "Application for allotting stocks to central depository account" and "Application for remitting cash dividends to bank account", and then mail them back to the company for registration. Stock dividends/ Cash dividends will be directly deposited to your designated account on the payment date of future dividends'distribution.

Draw the Cash Dividends
  1. Specimen Chop
  2. Application of drawing the Cash Dividends (You can obtain the application by calling 886-2-2500-1668)
  3. A piece of Specimen Chop Card (Not necessary for an original shareholder)
  4. A copy of National Identity Card (Including the front and the back side)(Not necessary for an original shareholder)

  1. Specimen Chop
  2. Application of drawing the Stock Certificates (You can obtain the application by calling 886-2-2500-1668)
  3. A piece of Specimen Chop Card (Not necessary for an original shareholder)
  4. A copy of National Identity Card (Including the front and the back side)(Not necessary for an original shareholder)

Note :

  • If you have payable tax, please bring the tax by Cash/ Check/Draft when drawing the Stocks.

 Relevant Matters about the Diividend Check
1. Specimen Chop
2. A piece of Specimen Chop Card (Not necessary for an original shareholder)
3. A copy of National Identity Card (Including the front and the back side)(Not necessary for an original shareholder)
4. Apply for the Cancellation of Prohibition of Endorsement:
  (1) Application of Replacement Check / Cancellation of Prohibition of Endorsement
  (2) Check
5. Apply for the Replacement of the overdue Check:
  (1) Application of Replacement Check / Cancellation of Prohibition of Endorsement
  (2) Overdue Check
6. Apply for the Replacement of the lost Check:
  (1) Application of Replacement Check / Cancellation of Prohibition of Endorsement
  (2) Recognizance of the lost Check

‧ If your dividend check is lost, it cannot be replaced until one year later of the maturity day.
